In other words, ensure quality of care (QοС) consideration for the individual customer. This is achieved by evaluating ideas, practices and previous events and applying the learning achieved to future practice [3]. Evidence is forever changing in the light of new research, new technology, new ways, as well as old ideas and options put together in new ways. This is challenging as it means best practice can’t conclusively and finally be established [3,4].

i. Define the Question: Identify Question, Search and Screen Literature, Refine Question.
ii. Collect the Evidence.
iii. Critically Appraise the Evidence for Validity/Relevance.
iv. Integrate the Evidence and Patient Factors: Make & Carry Out the Decision.
v. Evaluate the Process: Results & Self-reflection (Figure 3).

Does Vitamin D Influence the Ovarian Reserve in InfertileWomen-A Short Communication by

Calcium homeostasis and bone density are 2 important functions of Vitamin D in human beings. To mediate its biologic functions, it utilizes Vitamin D receptors, that are present mostly in calcium regulating tissues like the bone and parathyroid glands [1]. Still finding Vitamin D receptors within the reproductive tissues like the ovary, endometrium, testis. Hypothalamus along with pituitary has stimulated the study of Vitamin D’s role in human reproduction [1]. Previous studies showed that Vitamin D can change the antimullerian hormone (AMH) signaling, follicle stimulating hormone (FSH) sensitivity and progesterone (P) Production and release in human granulose cells as per Irani M and Merhi Z [1]. Moreover, recently Chu et al 2018 carried out a systematic review and meta-analysis in women who were undergoing assisted reproduction showed that women having enough Vitamin D had >odds of getting a live birth (odds ratio 1.33) as compared to those who had deficient Vitamin D levels [2]. Hence with the presence of Vitamin D deficiency world over within women of reproductive age, it is imperative to study the effect of Vitamin D deficiency on ovarian physiology.

Doctors will boast that their patients are not shy and tell them everything but that is not how it is. Patients, especially men, are a hindrance to themselves. Erectile dysfunction or the more embarrassing bent knob of Peyronie’s is something most men fear to admit. Even when they do summon the courage to talk to the doctor the best they can do is euphemise. This tells the doctor that the problem is more than physical, it is also psychological. Treat with care. The patient will be referred to a urologist. The penis is unique. Nowhere else on the male is there anything similar and in the female, there is no equivalent and yet the failure of this inflatable organ shares medical problems with other organs. Explain to the patient that the penis is a balloon that inflates with blood and he may begin to understand. If the blood cannot flow into the penis, it will not expand. The blockage can be like blockages that can occur elsewhere in the body and these are issue in which a vascular surgeon specialises. Angiogenesis is the formation of new blood vessels and that is the clue to the cure. What helps with heart disease can also be applied to the penis.
